重庆市12-15岁中学生口腔健康知识、态度、行为的调查分析

【摘要】 目的:了解重庆市12-15岁中学生口腔健康知识、态度、行为的情况,为重庆地区口腔预防保健活动的开展提供参考信息与科学依据。方法:按照《第四次全国口腔健康流行病学调查方法》对重庆市城乡地区12-15岁中学生进行随机抽样,通过问卷调查方式获取其口腔健康知识、态度及行为等方面的情况,将中学生每日刷牙两次及其以上定义良好刷牙行为,对重庆市中学生的刷牙行为进行单因素及多因素分析,并参考Anderson模型分析中学生卫生服务利用的影响因素。结果:重庆市12-15岁中学生良好刷牙比例为39.7%,使用牙线及含氟牙膏的比例分别为8.8%、7.5%,口腔健康知识知晓率为58.9%,88.6%对口腔健康保持积极态度,有看牙经历的中学生为54.5%,在学校接受过口腔健康教育的比例为17.5%,经Logistic逐步回归分析,发现父亲学历、母亲学历、是否知晓口腔检查的必要性与刷牙行为有显著关联,通过Anderson模型分析得到父亲学历高、母亲学历高、有牙痛经历的中学生其口腔卫生服务利用率较高。结论:与全国第四次口腔流行病学调查的结果相比,重庆市12-15岁中学生口腔健康行为及态度较好,且中学生的口腔健康知识普遍较差,今后开展的口腔健康教育需尽可能地将有效刷牙、健康饮食、口腔定期检查的健康观念传递给重庆市中学生。此外,现阶段重庆地区的中学生仍然是以问题为导向的口腔就医模式,有必要提高中学生定期口腔健康检查的意识,尽可能地将中学生口腔健康检查纳入常规体检项目,并积极推动以学校为渠道,进行中学生口腔健康教育的开展。针对重庆市中学生口腔卫生行为存在的地区差异,今后口腔预防工作的重心需逐步从主城转移到远郊和农村。

【Abstract】 Objective: To know the status of oral health knowledge,attitude,behavior of 12-15 years old middle school students and provide a theoretical basis of oral dieases prevention in Chongqing area.Methods: based on the guideline for the Fourth National Oral Health Survey,A stratified multistage ramdom sampling design was applied and a total of 3902 12-15 years old children were selected,All examinees answered an oral health knowledge questionnaire.The single-factor and multi-factor analysis of middle school students’ brushing behavior in Chongqing was conducted,and the Anderson model was used to analyze the influencing factors of middle school students’ health service utilization.Results: 39.7% of the children brushed their teeth twice or more per day.The average correct rate of oral health knowledge was 58.9%,and the average oral health positive attitude was 88.6%.54.5% of children had dental experience,and 17.5% of children could obtain oral education in school.After Logistic stepwise regression analysis,it was found that the parental education and knowing the necessity for oral examination or not were significantly associated with children’s brushing behavior,besides,high parent’s degree and having toothache experience have a higher utilization rate of oral health services.Conclusion :Compared to the national level,the 12-15 years old children in Chongqing had better oral health behavior and attitude,but poorer oralhealth knowledge.In the future,oral health education should pass on the healthy concept of effective brushing,healthy eating,and regular oral examination to Chongqing middle school students.In addition,middle school students in Chongqing are still problem-oriented oral medical treatment models at this stage.It is necessary to raise the awareness of regular oral health examinations for middle school students,like incorporate oral health examinations for middle school students into routine physical examinations and promote oral health education in middle schools.In view of the regional differences in oral health behaviors among middle school students in Chongqing,the focus of oral prevention work in the future needs to gradually shift from the main city to the outer suburbs and rural areas.
