青少年抑郁症患者多导睡眠结构图与事件相关电位P300的相关性研究

【摘要】 目的:探讨青少年抑郁症患者认知功能损害与睡眠障碍之间的相关性,并分析认知功能损害与睡眠障碍之间的相互影响机制。方法:对34例临床诊断为心境障碍(抑郁发作)青少年患者在基线时评定临床因素(包括性别、年龄)、汉密尔顿抑郁量表(HAMD)、事件相关电位(ERPs)P300潜伏期和波幅、多导睡眠脑电监测(PSG)。同期12例健康青少年人群进行汉密尔顿抑郁量表(HAMD)、事件相关电位(ERPs)P300潜伏期和波幅、多导睡眠脑电监测(PSG)。采用t检验、X~2检验、Pearson相关性分析、线性回归分析,使用IBM SPSS24.0软件完成数据统计。结果:1.患者组HAMD总分、睡眠时间、入睡后清醒时间、睡眠效率、睡眠潜伏期、REM期潜伏期、清醒期持续时间、REM期持续时间、N2期持续时间、N1期百分比、P300潜伏期和P300波幅与健康对照组相比差异有统计学意义,P<0.05。患者组清醒次数、N1期持续时间、N3期持续时间、REM期百分比、N2期百分比和N3期百分比与对照组相比无统计学意义,P>0.05;2.受试者P300潜伏期和波幅均与总睡眠时间、入睡后清醒时间、睡眠效率、清醒持续时间、REM期持续时间和REM期百分比呈相关性,P300潜伏期和P300波幅进入以HAMD总分为因变量的回归方程。结论:1.青少年抑郁症患者存在广泛的认知功能损害与睡眠障碍;2.青少年抑郁症患者的认知功能障碍与睡眠障碍相互影响,相互关联;3.P300潜伏期和波幅与快速眼动REM期呈相关性。

【Abstract】 objective: To explore the correlation between cognitive impairment and sleep disorders in adolescents with depression,and to analyze the interaction mechanism between cognitive impairment and sleep disorders.Methods: A total of 34 adolescent patients with clinically diagnosed mood disorder(depressive episodes)were assessed for clinical factors(including gender,age),Hamilton Depression Rating Scale(HAMD),event-related potential(ERPs)P300 latency and amplitude,and polysomnography at baseline.(PSG).In the same period,12 healthy adolescents were subjected to Hamilton Depression Scale(HAMD),event-related potential(ERPs)P300 latency and amplitude,and polysomnography(PSG).Data analysis was performed using IBM SPSS24.0 software using t test,X2 test,Pearson correlation analysis,and linear regression analysis.Results: 1.HAMD total score,sleep time,wakefulness after sleep,sleep efficiency,sleep latency,REM latency,duration of awake duration,duration of REM duration,duration of N2,percentage of N1 phase,P300 latency and P300 amplitude There was a statistically significant difference in the healthy control group,P<0.05.The number of awake,duration of N1,duration of N3,percentage of REM,percentage of N2,and percentage of N3 were not statistically significant compared with the control group,P>0.05.2.Subject P300 latency and amplitude were both.Correlation withtotal sleep time,wakefulness after sleep,sleep efficiency,duration of awake,duration of REM,and percentage of REM,P300 latency and P300 amplitude entered the regression equation with the HAMD total score.Conclusion: 1.Adolescents with depression have extensive cognitive impairment and sleep disorders;2.Adolescents with cognitive impairment and sleep disorders interact and correlate;3.P300 latency and amplitude and rapid eye movement REM Correlation.
